2023-24 FORMS & HANDBOOK

All Competitive Marchers (including Color Guard and Dancers), Non-Competitive members, and Concert Band-

only members must fill out the registration packet through Rank One for the 2023-2024 season. 

 

The link to Rank One (Online Registration System) will be emailed in May along with information on the process. NEISD participation forms along with UIL medical history and UIL physical forms must be completed annually and be on file with the Johnson Band before your student participates in any practice or rehearsal before, during, or after school, including games or competitions.

BEFORE YOU START

Please make sure you have the following information ready before you begin this process: 

  • Student ID

  • Parent Contact Information

  • Insurance Information 

TO BEGIN

  1. Click here to begin the registration process: NEISD Fine Arts / Parent Page (neisd.net) 

  2. Next, click on the High School & Middle School Online Participation Form Link 

  3. Proceed to Online Forms after reading all instructions 

  4. You will be prompted to Create an Account or Continue as Guest 

  5. Make sure to select Band (C) in the section that asks for your student's Currently Selected/Activities if it has not already been selected for you

  6. Fill out all forms completely and upload your student's physical along with their Medical History Form 

 

PLEASE NOTE: If your student is having their physical done at Johnson High School in May, they will take home their original paperwork for you to upload.  

FORMS TO BE SUBMITTED THROUGH RANK ONE

  1. Fine Arts Code of Conduct - You will be asked to click on the link to read the information before you can submit. The last page will ask you to "Please complete and return to the Fine Arts Department", YOU DO NOT need to. Submitting the form stating you read the Code of Conduct is all we need.

  2. Medication Release Form 

  3. Parent Travel Consent Form and Supplement to Parent Travel 

  4. Student Media Release Form - Please print the student's name in the box below the note that states "Please print student's name on the line above" 

  5. Travel Consent/Health Form 

  6. UPLOAD Pre-participation Physical Evaluation - This should be a PDF or picture of the Physical and Medical History Form

  • This process can take about a week and must be completed before volunteering

PHYSICAL CLINIC INFORMATION

  1. We will be hosting the Schertz/Cibolo Clinic at Johnson High School to do physicals for any band member needing one. COST IS $15 — CASH ONLY.

  2. Our clinic date is Thursday, May 26. If your student is under the age of 18, you will need to provide the parent release form provided by the Schertz/Cibolo Clinic. Make sure to add the correct date and year to the form. 

  3. Don't forget to bring your NEISD Physical and Medical History and Parent Release form FILLED OUT to the Clinic. If your student wears glasses or contacts, please make sure they wear those as well.

IMPORTANT DATES TO REMEMBER

  • Registration Begins: Saturday, May 21, 2022

  • Rank One Paperwork Due Dates:

    • Monday, July 15: Competitive Guard/Drumline

    • Monday, July 25: ALL other Competitive Band Members 

    • Monday, August 15: Non-Competitive, Concert Only, and Move-ins

​

All forms will be electronically submitted, including the NEISD Medical History Form and School Physical.
